29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: Employees working on the bunger platform and the shaver platform were exposed to falling approximately 4.5 feet and 6 feet to the concrete below because the area was not protected by a standard railing.

29 CFR 1910.27(d)(3): The employer did not ensure that the side rails of a through ladder extended 3 1/2 feet above parapets and landings, omitting the rungs from the extension and having not less than 18 nor more than 24 inches clearance between the rails. Employees working on the skinner platform were exposed to trip and fall hazards when ascending and descending the fixed ladder to access the platform because the ladder did not have side rails that extended 3.5 feet above the landing.
