29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Thermoforming Department, 303 Brush Creek Road, Warrendale, Pennsylvania 15086: Observed on or about November 25, 2013, employees were using two Tennsmith brand 52 inch shears that were not equipped with front hold down guards. The employees were exposed to amputation hazards while using the shears to trim excess material from thermoform products.
