29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) Facility, CNC Department, 303 Brush Creek Road, Warrendale, PA: On or about July 12, 2019 the point of operation on the Thermwood CNC machine, Model M90-105, Serial No. C90S0700215, was not guarded to prevent employees having any part of their body in the danger zone during operation. An employee leaned over the table to clear debris from a cutting tool when the machine cycled and struck the employee on the left side of their head with a sawing tool. The employee sustained a laceration, concussion and fractured skull.
