29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Employee(s) working at or near the Emerson motor for the no bake foundry drag line conveyor system were exposed to a struck by hazard due to a broken motor fan guard. The lower section of the fan guard had a 6 inch by 5 inch opening that exposed a rotating fan that operates at approximately 1745 revolutions per minute. (b) Employee(s) working at or near the east side of the material roll out barrel were exposed to a struck by hazard due to a 56 inch opening that allowed workers to come into inadvertent contact with a rotating material roll out barrel.

29 CFR 1910.219(c)(2): All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, were not protected by a stationary casing enclosing shafting completely: (a) An open section within a guard for the rotating shaft of transfer car #11801 measured 1 inch by 7 inch at the north end of the no bake foundry. Employee(s) were exposed to struck by hazards due to inadequate guarding for a rotating shaft spinning at approximately 1725 revolutions per minute. (b) Employee(s) operating the Pacemaster mixer #3822 were exposed to struck by hazards due to two rotating shafts above the control panel in the Core department were not being guarded adequately. The guard for the the left side rotating shaft had a 1 inch wide opening and the guard for the right side rotating shaft had a 2 inch wide opening. (c) Employee(s) operating the bench grinder #3903 in the Rod & Chill department were exposed to struck by hazards due to no guard being provided for the rotating motor shaft of the bench grinder. The unguarded rotating shaft on the left side of the bench grinder measured 1 inch by 3 inch and operated between 9500 - 12500 surface feet per minute.

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o): (a) Employee(s) working at the Iron oxide delivery platform were exposed to struck by hazards due to an overhead 14 inch diameter rotating pulley that was approximately 68 inches from the ground floor, not being guarded to prevent inadvertent contact with workers. The rotating pulley was running at approximately 1170 revolutions per minute. (b) Employee(s) working at or near the 6301 shaker on the north side of the 6300 panel were exposed to struck by hazards due to the guard in place having a 40 inch wide opening, thus allowing workers access to a rotating pulley. (c) Employee(s) using a passageway that was underneath the 6303 conveyor at the south side of the sand system conveyor to access the material roll out barrel were exposed to struck by hazards. There was no guard being provided for the rotating pulleys underneath the conveyor. The rotating pulleys were approximately 80 inches from the floor. (d) Employee(s) using the stairway to gain access to the basement level of the sand system were exposed to struck by hazards due to no guard being provided for the 6303 link belt pulley that was approximately 10 inches away from the stairway. The 6303 link belt pulley was approximately 50 inches above the stairs.

29 CFR 1910.219(e)(1)(i): Horizontal belts which had both runs seven feet or less from the floor level were not guarded with a guard that extended to at least fifteen inches above the belt: (a) Employee(s) working at the Iron oxide delivery platform were exposed to struck by hazards due to an overhead rotating belt that was between 55 - 68 inches from the ground floor, not being guarded to prevent inadvertent contact with workers. The rotating belt was running at approximately 1170 revolutions per minute. (b) Employee(s) working at or near the 6301 shaker on the north side of the 6300 panel were exposed to struck by hazards due to the guard in place having a 40 inch wide opening, thus allowing workers access to a rotating belt. (c) Employee(s) using a passageway that was under the 6303 conveyor at the south side of the sand system to access the material roll out barrel were exposed to struck by hazards. There was no guard being provided for the overhead rotating conveyor belt. The overhead rotating belt was approximately 80 inches from the floor. (d) Employee(s) using the stairway to gain access to the basement level of the sand system were exposed to struck by hazards due to no guard being provided for the 6303 link belt that was approximately 1 inch away from the stairway. The 6303 link belt was approximately 50 inches above the stairs.

The 7 inch by 5 inch opening exposed a rotating chain and sprocket. (c) Employee(s) working in the no bake foundry near the mechanized / mold line were exposed to caught between hazards due to no guard being provided for a 39 inch by 7 inch opening for the 1128 transfer car. The 39 inch by 7 inch opening exposed a rotating chain and sprocket.

29 CFR 1910.212(a)(5): Fan blade guards, where the periphery of the blades was less than seven feet above the floor or working level, had openings larger than one half inch: (a) Employee(s) working in the no bake foundry had access to an industrial fan on the upper platform mezzanine were exposed to amputation hazards due to a 3 inch by 6 inch opening on the front side and a 11 inch by 12 inch opening on the back side of the industrial fan not being guarded. The operational motor speed of the industrial fan was approximately 1740 revolutions per minute. (b) Employee(s) using an industrial fan in the Riser Burn area were exposed to amputation hazards due to 2 damaged areas within the front side of the industrial fan guard that allowed openings greater than quarter inch. The damaged areas measured approximately 1 inch by 2 inch and 1 inch by 3 inch. The operational motor speed of the industrial fan was approximately 1740 revolutions per minute. (c) Employee(s) using the industrial fan in the Arc Air area were exposed to amputation hazards due to a 1 inch diameter opening at the center of the front side of the fan guard. The operational motor speed of the industrial fan was approximately 1740 revolutions per minute. (d) Employee(s) using an industrial fan in the Melting department were exposed to amputation hazards due to a 1 inch by 7 inch opening on the front side of the industrial fan. The operational motor speed of the industrial fan was approximately 1740 revolutions per minute.
