29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. On or about December 5, 2013, two employees were observed working on the top-plate and outer edges of the house under construction without utilizing any fall protection system. Employees were exposed to a fall hazard of greater than six feet.

On or about December 5, 2013, located behind the house under construction, employees used a 10-foot, Werner A-frame ladder in a manner other than the purpose for which it was designed to access the roof. Employees were exposed to a fall hazard of greater than six feet.
