29 CFR 1910.27(d)(3): The side rails of through or side-step ladder extensions did not extend 3 1/2 feet above paraphets and landings:  a)  Exterior rear of building, on or about December 11, 2013, employees used a fixed ladder to gain access to the top of the silo.  The ladder did not extend 42 inches above the top of the silo, exposing employees to a fall hazard of 8 feet.   ABATEMENT CERTIFICATION IS REQUIRED.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a)    Blow mold production area,  on or about  December 4, 2013, injection blow mold machine #2 was not adequately guarded in that the gate guards on the sides of the machine could be easily opened without special tools, and was not interlocked to automatically shut the machine down when the gates were opened, exposing employees to a pinch point hazard.       b)      Blow mold production area,  on or about  December 4, 2013, Impco injection blow mold machine #2 was not adequately guarded in that the gate guards on the sides of the machine could be easily opened without special tools, and was not interlocked to automatically shut the machine down when the gates were opened, exposing employees to a pinch point hazard.  c)     Blow mold production area, on or about December 4, 2013, Rocheleau injection blow mold machine #5 was not adequately guarded in that the area where the two halves of the mold come together was not guarded on top, exposing employees to a pinch point hazard.    d)    Blow mold production area, on or about December 4, 2013, injection blow mold machine #6 was not adequately guarded in that the area where the two halves of the mold come together was not guarded on top, exposing employees to a pinch point hazard.    e)     Blow mold production area, on or about December 4, 2013, injection blow mold machine #7 was not adequately guarded in that the area where the two halves of the mold come together in that the gate guards on the sides of the machine could be easily opened without special tools, and was not interlocked to automatically shut the machine down when the gates were opened, exposing employees to a pinch point hazard., exposing employees to a pinch point hazard.    f)      Blow mold production area, on or about December 4, 2013, injection blow mold machine #11 was not adequately guarded in that the area where the two halves of the mold come together was not guarded on top, exposing employees to a pinch point hazard.      g)    Blow mold production area, on or about December 4, 2013, Rosade blow mold machine #12 was not adequately guarded in that the area where the two halves of the mold come together was not guarded on top, exposing employees to a pinch point hazard.      h) Behind Blow Mold machine #12, the area where the conveyor belt went over the bottom drum      near the floor was not guarded, exposing employees to an in-running nip point hazard.    i) Blow Mold machine area, on or about January 30, 2014, the fire (flame) from the flamer machine was not guarded, exposing employees to a burn hazard.    ABATEMENT CERTIFICATION AND DOCUMENTATION IS REQUIRED.
