Melville NY - An employee was walking and working in the vicinity of moving vehicles and equipment while cleaning the back of a 100 cubic yard walking floor trailer.  The employee was not wearing a reflectorized vest and was struck by an 836H Landfill Compactor; on or about 12/9/13.    b)  Worksite, 136 Spagnoli Rd. Melville NY - An employee was walking and working in the vicinity of moving vehicles and equipment while cleaning the back of a 100 cubic yard walking floor trailer.  The employee was not wearing a reflectorized vest and was exposed to being struck by moving vehicles and equipment; on or about 12/30/13.    C)  Worksite, 136 Spagnoli Rd. Melville NY - An employee was removing a tarp from a 100 cubic yard trailer while walking and working in the vicinity of moving vehicles and equipment.  The employee was not wearing a reflectorized vest and was exposed to being struck by moving vehicles and equipment; on or about 3/25/13.        Note:  In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
