29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of Iron Oxide listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 10 milligrams per cubic meter:     a)  At the facility, on or about December 9, 2013 - An employee designated as a welder was exposed to Iron Oxide to an airborne concentration of 18.4 milligrams per cubic meter.  This is approximately 1.84 times in excess of the permissible exposure limit of 10 milligrams per cubic meter.  Sampling was conducted for 198 minutes and zero exposure was assumed for 282 minutes.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:    a)  At the facility on or about December 9. 2013 -  Employees were not provided training on the new label elements and safety data sheets format.  Employees are exposed to welding fumes including, but not limited to, iron oxide when welding on carbon steel.
