29 CFR 1910.134(e)(6): In determining the employee's ability to use a respirator, the employer shall obtain a written recommendation regarding the employee's ability to use the respirator from the PLHCP (physician or other licensed health care professional): (a) Paint area, 101 Port Ambridge Drive, Ambridge, PA- On or about March 3 2015, all employees wearing a tight fitting, half-face cartridge respirator were not provided with a medical evaluation to determine the employees ability to use a tight fitting respirator.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (a) Paint Area- 101 Port Ambridge Drive, Ambridge, PA: On or about February 26, 2016, all employees working with and exposed to chemicals such as but not limited to Xylene and Kem Bond Universal Metal Primer were not provided with information and training on working with such chemicals.
