29 CFR 1910.304(g)(5): Grounding path. The path to ground from circuits, equipment, and enclosures was not  permanent, continuous, and effective.     This violation was discovered on December 9, 2013; where employees worked with and around the following equipment which was not properly grounded:        a) One Festool, CT 33, Dust Extractor.       b) One Stinger 2.5 gallon Wet/Dry vacuum.

29 CFR 1910.305(g)(2)(ii): Flexible cords may be used only in continuous lengths without splice or tap. Hard-service cord and junior hard-service cord No. 14 and larger may be repaired if spliced so that the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced.          This violation was discovered on December 9, 2013; where maintenance employees worked with and around a damaged 120 volt Hitachi cutoff saw. The flexible cord to the motor was spliced with uncovered plastic wire nuts. The plastic wire nuts were subjected to strain imposed by a dust collection bag that rubbed on them. Employees that worked with and around the cutoff saw were thereby exposed to an electrical hazard.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment had been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment:       This violation was discovered on December 9, 2013; where employees were required to work with hazardous chemicals such as EcoLab Solar Brite, and EcoLab Fast Form Degreaser. The employee's skin and eye were exposed to the hazards and necessitated the uses of personal protective equipment.  The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed.

The employer did not have in place and/or in writing a safety-related work practices program as required by 29 CFR 1910.333(b)(l).
