29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, the use of a safety monitoring system alone was not used:   a. West side of building - employees were exposed to a 11'-2" fall hazard, in that, the use of fall protection was not enforced while working from the flat portion of the roof near the edge. Violation observed on or about 12/09/13.  Dempsey Roofing Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 315437418, citation number 1, item number 1, and was affirmed as final order on 04/01/11, with respect to a workplace located on 7601 S. Orange Blossom Trail, Orlando, FL 32809

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. South side of building - employees were exposed to a 13'-10" fall hazard, in that, the ladder did not extend 36" above the roof landing. The ladder extended approximately 30" above the landing. Violation observed on or about 12/09/13.
