As a result, employees were exposed to slip, trip and fall hazards from the water accumulation and the drainage ditch adjacent to the cutting table area. Any deficiencies such as listed shall be carefully examined and determination made as to whether they constitute a safety hazard: (a) On December 11, 2013, at the facility located at 2100 Manchester Traffic Way, Kansas City, MO., The employer had not performed any complete Periodic Inspections of the Harrington 1 Ton Crane and Hoist operated in the Main Shop area. As a result, employees were exposed to potential struck-by injuries from the crane operations. The Pendant Control Cable to the Harrington 1 Ton Crane and Hoist in the main shop area; and 2. The Extension Cord used with the portable heater in the rest room area. As a result, employees were exposed to electrical shock and burns and struck-by injuries if the over head crane were to short and possibly drop a load.
