29 CFR 1910.22(a)(2): Where wet processes were used, drainage was not maintained, and false floors, platforms, mats, or other dry standing places were not provided when practicable.     (a) On December 11, 2013, at the facility located at 2100 Manchester Traffic Way, Kansas City, MO., The employer did not ensure that adequate water drainage was provided and maintained as a result of the wet cutting processes associated with the Granite Slab cutting activities. As a result, employees were exposed to slip, trip and fall hazards from the water accumulation and the drainage ditch adjacent to the cutting table area.            Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1910.179(j)(3): Periodic inspection. Complete inspections of the crane shall be performed at intervals as generally defined in paragraph (j)(1)(ii)(b) of this section, depending upon its activity, severity of service, and environment, or as specifically indicated below. These inspections shall include the requirements of paragraph (j)(2) of this section and in addition, the following items. Any deficiencies such as listed shall be carefully examined and determination made as to whether they constitute a safety hazard:     (a) On December 11, 2013, at the facility located at 2100 Manchester Traffic Way, Kansas City, MO., The employer had not performed any complete Periodic Inspections of the Harrington 1 Ton Crane and Hoist operated in the Main Shop area. As a result, employees were exposed to potential struck-by injuries from the crane operations. No nozzle pressure reducing device had been installed to ensure no more than 30 P.S.I. was being used. 29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws    (a) On December 11, 2013, at the facility located at 2100 Manchester Traffic Way, Kansas City, MO., The employer did not ensure that strain relief was provided on the flexible cords in the following instances:          1. The Pendant Control Cable to the Harrington 1 Ton Crane and Hoist in the main shop area; and         2. The Extension Cord used with the portable heater in the rest room area.     As a result, employees were exposed to electrical shock and burns and struck-by injuries if the over head crane were to short and possibly drop a load.
