Employer did not obtain manufacturer's written approval before installing Cascade clamps on the following forklift trucks: - 1999 Crown SC (Model #35SCTT) - 2008 Crown RC (Model #RC5540-40TT) Condition observed on or about November 25, 2013. Wakefern Food Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(a)(4), which was contained in OSHA inspection number 157293, citation number 1, item number 2, and was affirmed as a final order on 2/20/2009, with respect to a workplace located at 5000 Riverside Drive, Keasby, NJ 08832.
