29 CFR 1910.22(a)(2): The floor of every workroom was not maintained in a clean and, so far as possible, a dry condition. Where wet processes where used, adequate drainage was not maintained, and false floors, platforms, mats, or other dry standing places where not provided where practicable. (a)Southwest Airlines at 8500 Pena Blvd., Denver, CO 80249; On and before December 12th, 2013, the exposing employer did not ensure that the floor of every work room was kept in a dry condition. Specifically, the triturator waste disposal room at DIA concourse C, was not kept clean and in a dry condition due to a continuous grease leak from a drain pipe on the ceiling, which combined with cold temperatures to generate an exteremly slick surface on which to work. The condition exposed employees utilzing the room to slipping hazards attributed to grease accumulation. (b)Southwest Airlines at 8500 Pena Blvd., Denver, CO 80249: On and before January 1st, 2013, the exposing employer did not ensure ice and snow accumulation had been removed in areas where employees walked on the ramp on Concourse C. Three employees were injured when they slipped and fell on the same ramp on the C concourse of Denver International Airport, due to ice and snow accumulation which was not attended to in an expeditious manner. The condition exposed employees working the tarmac to slip and fall as they attended to aircraft.

29 CFR 1910.22(a)(3): Every floor, working place, and passageway were not kept free from protruding nails, splinters, holes, or loose boards. (a)Southwest Airlines at 8500 Pena Blvd., Denver, CO 80249: On and before December 12, 2013, the exposing employer did not ensure that floors were kept free from raised flooring material that presented a tripping hazard. Specifically, employees were exposed to trip hazards from loose, raised, rubber mats while unloading baggage from the conveyor systems onto carts attached to tugs in the T-point bagge handling area under the airport. The condition exposed employees to trip hazards while handling luggage.
