29 CFR 1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):   On or about June 20, 2022 and times prior thereto, workers were exposed to a fall hazard due to unprotected edges at the top platform of self-propelled mobile stairways while positioning against aircraft.

29 CFR  1910.30(a)(1): The employer did not provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart before any employee is exposed to a fall hazard. And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.   On or about June 20, 2022, and times prior thereto, workers were exposed to a fall hazard without training on the use of personal fall protection systems when setting up and utilizing mobile stairways to access an aircraft.

29 CFR  1910.132(d)(1):The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  On or about June 20, 2022, and times prior thereto, workers were exposed to slip/fall hazard due to lack of hazard assessment for outdoor weather conditions, e.g., ice, rain, etc., on wet walking working surfaces on mobile stairways.
