OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a) On or about December 16, 2013 the indoor filter media-type dust collector units (Kelva systems) used for collecting combustible paper dust were not provided with a means of: i. Explosion protection for the dust collectors ii. Fire protection for the dust collectors iii. Deflagration (flame front) propagation protection between the dust collector units and the upstream process equipment

29 CFR 1910.307(c): Equipment, wiring methods, and installations of equipment in hazardous Class II locations were not intrinsically safe, approved for the hazardous Class II location, or safe for the hazardous Class II location: a) Employees were exposed to hazards associated with unapproved electrical equipment in a Class II hazardous area in the napkin converting area of the facility. A motor for a conveyor and an electrical outlet was found in the Class II area of the facility. ABATEMENT DOCUMENTATION REQUIRED
