29 CFR 1910.134(f)(2):   The employer did not ensure that an employee using a tight-fitting facepiece respirator was fit tested prior to initial use of the respirator, whenever a different respirator facepiece size, style, model or make) was used, and at least annually thereafter:  On or about 12/17/2013, an employee engaged in screen printing/cleaning operations wearing a Protech  Half-Face Respirator and NIOSH G100 Filters, had not been fit tested.

29 CFR 1910.134(g)(1)(i)(A):   The employer permitted respirators with tight-fitting facepieces to be worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with the valve function:  On or about 12/17/2013, an employee who performs screen printing/cleaning operations had facial hair that can result in an improper seal of the respirator, exposing him to hazards.

29 CFR 1910.212(a)(1):   One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about 10/14/2013, in the Pad Room Department, Comec Pad Printer, Model #60LPEC, Serial #0173, was being operated by an employee without a guard.  The hazard created by a pinch point resulted in injuries to the employee.
