29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  (a) Roof, new residential construction located at 3378 Ebenezer Farm Road, Marietta, GA 30062: Employees were exposed to potential eye injuries while operating pneumatic Hitachi (Model - NV45AB2) Coil Roofing Nail Guns without eye protection. Employees firing 1-1/4-inch wire collated coil roofing nails at 120 PSI were not provided with safety glasses or goggles.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) Roof, new residential construction located at 3378 Ebenezer Farm Road, Marietta, GA 30062: Employee was exposed to fall hazards while engaged in residential roofing activities on a 10/12 pitched roof at eave heights of approximately 23-feet with no fall protection system in place.

29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body narness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.  (a) Roof, new residential construction located at 3378 Ebenezer Farm Road, Marietta, GA 30062: Employee engaged in residential roofing work on a 12/12 pitched roof at heights of approximately 15-feet, 6-inches from the lower level was wearing a mountain climbing harness with the attachment to the life-line located on the front side waist exposing the employee to potential back injuries in the event a fall occurred as the personal fall arrest system provided was not used correctly. The attachment point of the bolt belt/lanyard was not located in the center of the wearer's back.  (b) Roof, new residential construction located at 3378 Ebenezer Farm Road, Marietta, GA 30062: Employee engaged in residential roofing work on a 10/12 pitched roof at heights of approximately 30-feet from the lower level was wearing a mountain climbing harness with the attachment to the life-line located on the front side waist exposing the employee to potential back injuries in the event a fall occurred as the personal fall arrestsystem provided was not used correctly. The attachment point of the bolt belt/lanyard was not located in the center of the wearer's back.
