Employees were observed exposed to fall hazards of up to approximately 8 feet, 3 inches while walking/working on the mezzanine in areas where openings in the guardrail system of up to approximately 24 inches existed.     b) Job site, 4 Pennwest Way, State Route 38, Emlenton, PA: On or about December 19, 2013, during production operations on a catwalk mezzanine in the roofing/siding section of the production floor, the employer did not ensure that an appropriate standard railing system existed where employees were exposed to falls of up to approximately 7 feet existed. The employer had a railing system along the edge of the catwalk mezzanine that was approximately 28 inches tall.
