29 CFR 1910.178(n)(7)(i): Loaded industrial truck(s) ascended or descended grades over 10% with the load downgrade:  At the time of the inspection, it was determined that the employee was moving a walking floor trailer from Bay #5 to Bay #1 downgrade when the load slipped off the forklift and travelled down the 15% ramp into the wash building. There was no method in place to properly secure the load to the forks. There was not room inside the building to allow the trailer to be turned around so the load could be moved with the load upgrade.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE)  At the time of the inspection, the employer had not conducted a PPE assessment for the paint mixer who did not wear a respirator while mixing paints and additives in the paint kitchen.
