29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by the use of a guardrail system, safety net system, personal fall arrest system, warning line system, or a safety monitoring system. 5400 E. Texas St., Bossier City, LA: On or about November 19, 2013, the employer failed to ensure that employees performing roofing operations at a height of over 6 ft, wore adequate fall protection. The employees were installing counter flash on a roof with a 3:12 pitch without wearing fall protection, exposing themselves to a fall hazard.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards as specified in 1926.503(a)(2) through (a)(2)(vii).. 5400 E.
