29 CFR 1910.132(f)(1): The employer shall provide training to each employee who is required by this section to use PPE. Each such employee shall be trained to know at least the following:  29 CFR 1910.132(f)(1)(iii): How to properly don, doff, adjust, and wear PPE;  a.  At the workplace, the employer did not provide training to employees required to wear fall protection while performing work on top of presses, exposing employees to a fall hazard.

29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  29 CFR 1910.147(c)(4)(ii)(B): Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy;  a.  At the workplace, in the bottle room, the Blister Pack Machine did not have specific energy control procedures, exposing employees to an amputation hazard.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.  a.  At the workplace, in the bottle room, the line two rapid pack robot guard had an opening approximately 10 inches by 52 inches therefore, not adequately guarded exposing employees to a struck-by hazard.  b. At the workplace, the Nissei injection molding machine, model #FN7000, serial#S362091, had a guard that allowed an employee to access the injector pin therefore, not adequately guarded exposing employees to an amputation hazard.  On or about December 9, 2013, an employee suffered an amputation while working on the machine.
