29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.  a.  At the workplace, in the bottle room, the line two rapid pack robot guard had an opening approximately 10 inches by 52 inches therefore, not adequately guarded exposing employees to a struck-by hazard.  b. At the workplace, the Nissei injection molding machine, model #FN7000, serial#S362091, had a guard that allowed an employee to access the injector pin therefore, not adequately guarded exposing employees to an amputation hazard.  On or about December 9, 2013, an employee suffered an amputation while working on the machine.
