29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) Production: The employer did not develop, document and utilize procedures for the control of potentially hazardous energy while employees were engaged in service and maintenance activities of the mulit cross-cut saw, observed on or about December 20, 2013.    Abatement Certification Required.    ABATEMENT NOTE:    The procedures should clearly and specifically outline the scope, purpose, authorization, rules and techniques to be utilized for the control of hazardous energy, the means to enforce compliance including but not limited to:    A specific statement of the intended use of the procedure.    Specific procedural steps for shutting down, isolating, blocking and servicing machines or equipment to control hazardous energy.    Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them.    Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices and other energy control measures.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:   a) Production:  On or about December 20, 2013, the employer did not provide training to ensure that the purpose and function of the energy control program was understood and that the knowledge and skills required for the safe application, usage and removal of the energy controls are acquired by maintenance employees, equipment operators and other employees.    Abatement certification required.   ABATEMENT NOTE:    Training must be tailored to the requirements of the job responsibility:   Employees conducting servicing and maintenance on equipment must be trained as authorized employees as per 29 CFR 1910.147(c)(7)(i)(A)   Employees operating equipment which may be locked out must be trained as affected employees as per 29 CFR 1910.147(c)(7)(i)(B)   All other employees must be trained as per 29 CFR 1910.147(c)(7)(i)(C)

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 psi:       a) Production:  Compressed air was used by employees to blow wood dust from the wooden pallets and to remove dust from their clothing at more than 30 psi, exposing employees to possibly bodily injury, on or about December 20, 2013.       M & M Wood Products was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1910.242(b), which was contained in OSHA inspection number 314160797 Citation Number 01, Item number 005, and was affirmed as a final order on February 18, 2011, with respect to a workplace located at 159 Lake Street, Morris Run, PA 16939.                                                             ABATEMENT DOCUMENTATION REQUIRED.
