29 CFR 1910.305(a)(2)(x): Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points. (a) On or about 12/11/13, at the Willow Beach National Fish Hatchery, Shed 117, an orange flexible cord, used to extend power to the formaldehyde storage shed, was routed through the doorway and subject to a pinch point.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries. (a) On or about 12/11/13, at the Willow Beach National Fish Hatchery, maintenance area, a portable fire extinguisher was left free standing on top of a work bench. The US Fish and Wildlife Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(c)(1), which was contained in OSHA inspection number 307503896, citation number 3, item number 2 and was affirmed as a final order on 4/13/09, with respect to a workplace located at US fish and Wildlife Service, Kenai National Wildlife Refuge, Kenai, AK 99669.
