29 CFR 1910.1026(d)(2)(i): The employer using the scheduled monitoring option did not perform initial monitoring to determine the 8-hour time-weighted average exposure to chromium (VI) for each employee on the basis of a sufficient number of personal breathing zone air samples to accurately characterize full shift exposure on each shift, for each job classification, in each work area. Location - Plating Area: On or about December 27, 2013, the employer had sampled to determine employees' exposures to chromium but had not taken steps to determine specifically the exposures to hexavalent chromium Cr(VI).

29 CFR 1910.1026(d)(2)(vi): The employer did not perform additional monitoring when there had been a change in the production process, raw materials, equipment, personnel, work practices, or control methods that resulted or may have resulted in new or additional exposures to chromium (VI), or when the employer had any reason to believe that new or additional exposures had occurred. Location - Plating Area: On or about December 27, 2013, the employer did not perform additional monitoring when new processes involving hexavalent chromium had been added after their initial monitoring.
