29 CFR 1910.178(a)(5): If the truck is equipped with front-end attachments other than factory installed attachments, the user shall request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. Shipping/Receiving Area - On or about May 29, 2025, the employer did not request that the Toyota forklift, Model 50-8FGU32, Serial No. 10918 be marked to identify the 72 inch fork extensions along with approximate weight and capacity.
