29 CFR 1910.178(l)(4)(ii)(A): Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator has been observed to operate the vehicle in an unsafe manner:    (a)  On or about December 20, 2013, a forklift operator was driving around the warehouse with the forks at knee level while employees are working in the area exposing them to struck-by hazards.  Refresher training was not provided before allowed to resume operations.    (b)  On or about December 20, 2013, a forklift operator was driving around the warehouse and hit the overhead lights exposing employees to struck-by and electrical hazards.  Refresher training was not provided before allowed to resume operations.
