At a commercial worksite located at 8050 N Church Rd, Liberty MO -- Employees engaged in steel framing and welding were not wearing eye protection. This exposed employees struck by or flash burn hazards. This was documented on or about 2/23/2024.

29 CFR  1926.453(b)(2)(v):29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  At a commercial worksite located at 8050 N Church Rd, Liberty MO -- Employee engaged in working from an aerial lift was not wearing a harness with lanyard attached to the basket. This exposed the employee to being catapulted from the aerial lift to ground level. This was documented on or about 2/23/2024.   Single Source Systems was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.453(b)(2)(v), which was contained in OSHA inspection number 1617168, citation number 1, item number 2 and was affirmed as a final order on 10/17/2022, with respect to a workplace located at 111 NW 121 Rd, Warrensburg, MO.
