29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: (a) Location: 791 Morgan Rd, West Springfield, MA: On or about December 30, 2013, four employees were observed performing roofing work on a steep roof exposed to a fall hazard 11 feet to the ground surface below without any fall protection. Dave Miner Exterior Home Improvements was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 313203663, citation number 1, item number 1 and was affirmed as a final order on March 28, 2010, with respect to a workplace located at 87 Felix Street, Chicopee, MA 01013.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) Location: 791 Morgan Rd, West Springfield, MA: On or about December 30, 2013, employees were using a ladder that extended less than 2 ft above the upper landing surface to which the ladder was used to gain access which is not in accordance with this subpart. Dave Miner Exterior Home Improvements was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.1053 (b)(1), which was contained in OSHA inspection number 313203663, citation number 1, item number 2 and was affirmed as a final order on March 28, 2010, with respect to a workplace located at 87 Felix Street, Chicopee, MA 01013.
