29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system: (a) 63 Osceola Ln Longmeadow, MA: On or about January 13, 2014, two employees were working on the roof of this residence more than 9 FT above the ground below without any fall protection as required by this subpart: Dave Miner Exterior Home Improvements was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501 (B) 13, which was contained in OSHA inspection number 313203663, citation number 1, item number 1and was affirmed as a final order on March 28, 2010 with respect to a workplace located at 87 Felix St Chicopee, MA.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access: (a) 63 Osceola Ln Longmeadow, MA: On or about January 13, 2014, an employee was climbing down from the roof of this residence using an aluminum ladder that did not extend 3 FT past the roof surface as required by this subpart: Dave Miner Exterior Home Improvements was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501 (B) 13, which was contained in OSHA inspection number 313203663, citation number 1, item number 2 and was affirmed as a final order on March 28, 2010 with respect to a workplace located at 87 Felix St Chicopee, MA.
