29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    Location:  578 Weld Street, W Roxbury, MA    Employees were exposed to fall hazards of over 20 feet while working to re-shingle a residential home where the employer did not adequately train the employees to recognize and avoid the hazards of not utilizing personal fall arrest systems.     THE COMPANY, LCT CONSTRUCTION INC., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.21(b)(2), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313802266, CITATION NUMBER 1, ITEM 1, ISSUED ON OCTOBER 27TH, 2010 WITH A FINAL ORDER DATE OF JANUARY 6th, 2011.
