29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment: On or about January 8, 2014 and times prior thereto, employees of Hackney Ladish, Inc. manufactured metal pipe fittings for oil and gas operations in the hot form facility located at or near 708 S. Elmira Avenue in Russellville, Arkansas. The employer had not selected and ensured employees use personal protective equipment for hazards identified in the following instances: a. Employees performing fabrication of metal pipe fittings were not provided with effective eye protection to protect from flying debris hazards, exposing employees to eye injuries. b. Employees performing welding and cutting operations on metal pipe fittings were not provided with appropriate personal protective equipment to include but not limited to flame retardant clothing, exposing employees to burns.

A vertical milling machine manufactured by Sharp with identification number MA447 was observed unguarded at the point of operation, exposing the employee to potential amputation hazards. b. A vertical milling machine manufactured by Cincinnati with identifcation number 5145 was observed unguarded at the point of operation, exposing employees to amputation hazards.

Brass shell, paper-lined sockets, or other metal-cased sockets may not be used unless the shell is grounded. On or about January 8, 2014 and times prior thereto, employees of Hackney Ladish, Inc. manufactured metal pipe fittings for oil and gas operations in the hot form facility located at or near 708 S. Elmira Avenue in Russellville, Arkansas. Lamp fixtures were installed on machinery and were not guarded to protect from accidental contact or breakage as follows: a. A lamp fixture was observed installed on the vertical milling machine manufactured by Sharp with identification number MA447. The lamp fixture was within 8 feet from the ground, unguarded with live parts exposed to accidental contact or breakage. b. A lamp fixture was observed installed on the vertical band saw manufactured by DoAll with identification number 9903. The lamp fixture was within 8 feet from the ground, unguarded with live parts exposed to accidental contact or breakage.
