29 CFR 1910.106(e)(6)(ii): Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 degrees F (37.8 degrees C), were dispensed into containers without the nozzle and container being electrically interconnected.  (a)  COLD STORAGE ROOM  On or about February 12, 2014, 1 gallon safety storage cans intended for use with flammable liquids were filled from a 55-gallon drum of Fast Dry Lacquer Thinner; a Category 2 Flammable Liquid with a flash point of -15 degrees F and containing toluene, methanol, and acetone; without the containers being electrically interconnected.  (b)  INSPECTION AREA  On or about February 12, 2014, one quart "JustRite Plungers" intended for use with flammable liquids were filled from one-gallon safety storage cans containing Fast Dry Lacquer Thinner; a Category 2 Flammable Liquid with a flash point of -15 degrees F and containing toluene, methanol, and acetone; without the containers being electrically interconnected.

29 CFR 1910.1200(f)(5): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged, or marked with the identity and appropriate hazard warnings regarding the chemicals contained therein:  INSPECTION/STAMPING AREA:  On or about February 12, 2104, JustRite Plungers with one-quart capacity containing lacquer thinner; a Category 2 Flammable Liquid consisting of toluene, methanol, and acetone; did not have labels, signal words, hazard statements, pictograms, or precautionary statements indicating the contents of the containers.  The name, address and telephone number of the manufacturer were also not present on the containers.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a)  MOLDING AREA   On or about February 12, 2014, the employer had not provided employees who work with molding compounds containing styrene with information and training regarding the physical and health hazards of working with these compounds.  Employees handle the molding compound with heat resistant gloves, place it in the molding machine, close the machine thereby compressing the compound, and baking the molding compound to a temperature of approximately 300 degrees F.   (b)  STAMPING AREA   On or about January 10, 2014, the employer had not provided employees who work with and lacquer thinner and liquid hardener containing toluene diisocyanate (TDI) with information and training regarding the physical and health hazards of working with the material.
