29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways and wherever turns or passage had to be made: a. On or about January 10, 2014, on the south train servicing ramp platform(s), in area(s) where mechanical handling equipment is used, multiple service line covers were broken, not mounted and out of place. Employees were exposed to trip hazards created by the multiple floor holes as a result of covers and concrete structural supports not being kept in good repair.
