29 CFR 1926.1408(a)(2): When the employer determined that any part of the equipment, load line or load (including rigging and lifting accessories), if operated up to the equipment's maximum working radius in the work zone, could come closer than 20 feet to a power line, they did not meet the requirements in Option(1), Option (2), or Option (3) of this section:  a) On or about 01/09/2014 at the job site, a 46 foot long luminaire 480 volt top mount light pole was being lifted with a Simon-Telelect truck crane to be attached to a base that was located approximately 12.5 feet horizontally from an adjacent power line, bringing the equipment, load line and load closer than 20 feet to the power line.  The employer did not meet the requirements in Option (1), Option (2), or Option (3) of this section.  The pole came loose from the crane hook and fell onto the power lines, exposing employees to an electrical shock hazard.

29 CFR 1926.1425(c)(2): When employees were engaged in hooking, unhooking, or guiding the load, or in the initial connection of a load to a component or structure, hooks with self-closing latches or their equivalent were not used:  a) On or about 01/09/2014 at the job site, a 46 foot long luminaire 480 volt top mount light pole was being lifted with a Simon-Telelect truck crane using a crane hook with a broken self-closing latch.  As the pole was lowered onto the base, it slipped off of the hook and fell onto an adjacent power line, exposing employees to an electrical shock hazard.
