29 CFR 1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in the trench excavation that was 4 feet or more in depth for employees to egress when required. Employees were exposed to engulfment hazards.  a. On or about March 10, 2021, at the site located on SR 528 mile marker 11.8, employees installing a manhole inside an excavation were exposed to engulfment hazards in that a proper means of egress was provided but not used.

On or about March 10, 2021, at the site located on SR 528 mile marker 11.8, employees installing a manhole in an excavation were exposed to a cave in hazards, in that; no adequate precautions were in place when accumulated water occupied the bottom of the excavation.

29 CFR  1926.416(a)(1):Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means.  a. On or about February 26, 2021, at the site located on State Road 528 eastbound near mile marker 11, employees were exposed to electrical shock when the employer failed to de-energize and ground or effectively guard circuits prior to allowing employees to move electrical wires during an installation of a man-hole box.  b. On or about March 2, 2021, at the site located on State Road 528 eastbound near mile marker 11, employees were exposed to electrical shock when the employer failed to de-energize and ground or effectively guard circuits prior to allowing employees to dig a trench, lay and splice wires that energize 6 street lights.

On or about February 3, 2021, at the site located on SR 528 mile marker 11.7, employees were exposed to cave in hazards when the employer failed to install protection for employees who were installing a man-hole box in an excavation of approximately 8 feet in width x 10 feet depth.  b. On or about February 4, 2021, at the site located on SR 528 mile marker 11.7, employees were exposed to cave in hazards when the employer failed to install protection for employees who were installing a man-hole box in an excavation of approximately 8 in width x 10 feet in depth.  c. On or about March 10, 2021 at the site located on SR 528, employees were exposed to cave in hazards when the employer failed to install protection for employees who were installing a man-hole box in an excavation of approximately 8 feet in width x 10 feet in depth.
