29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) At the Schmidt Die Cutters; The die cutters were not guarded to prevent other employees in the work area from hazards created by point of operation. An employee suffered an amputation at the press #68 die cutter. b) At the Bridgeport Milling Machines; The employees were not protected from rotating parts and flying chips as the employer did not have adequate guarding on the Bridgeport Milling Machines.
