29 CFR 1910.303(b)(7)(iv): There were damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemcial action or overheating. On or about 01/13/2014 at the 7310 Hall Street, St Louis, MO location, the dome lights hanging from the ceiling have exposed electrical wiring. The lights were flickering as a result of a light cover that was damaged when contact was made with stacks of pallets supported by the forks belonging to the forktruck.
