Guards were not affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard was not such that it does not offer an accident hazard in itself. , At 14916 Hwy 177, Jackson, MO, 63755, On August 23, 2023, and at time prior thereto, employees were exposed to hazards from an unsecured guard covering a chain and gear.   On August 17, 2023 an employee was working near the chain and gear when his right foot became caught between the chain and gear causing his small toe to be crushed and his next toe to have the tip amputated.
