The employee's December 3, 2013 annual audiogram indicated that the employee had suffered a suspected standard threshold shift, (STS) and was not referred for retesting, clinical audiological evaluation or an otological examination, January 13, 2014.  b) An employee, machine operator, was exposed to occupational noise above 85 decibels A-Scale.  The employee's December 4, 2013 annual audiogram indicated that the employee had suffered a suspected standard threshold shift, (STS) and was not referred for retesting, clinical audiological evaluation or an otological examination, January 13, 2014.
