29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: A) Pallet World, Inc., located at 8292 Fremont Pike, Perrysburg, OH 43551: On or about January 14, 2014, the employer failed to establish a lockout/tagout program for the control of the hazardous energy, which includes procedures, training, and periodic inspections, while performing activities such as, but not limited to, changing saw blades on equipment such as, but not limited to, the band saws located in the production area. Employees were exposed to amputation hazards while changing saw blades. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.213(r)(3): The point of operation on combination or universal woodworking machines was not guarded: A) Pallet World, Inc., located at 8292 Fremont Pike, Perrysburg, OH 43551: On or about January 14, 2014, the employer did ensure that adequate guarding was provided to protect employees from the saw blade of the band saw (built by Heartland Fabrication Machine, Inc.) located in the production area. The band saw did not have adequate guard that would prevent employees from accessing the point of operation, exposing employees to amputation hazards while operating the saw. B) Pallet World, Inc., located at 8292 Fremont Pike, Perrysburg, OH 43551: On or about January 14, 2014, the employer did ensure that adequate guarding was provided to protect employees from the saw blade of the Pallet Hawg band saw (built by Built With Skill, LLC) located in the production area. The band saw did not have adequate guard that would prevent employees from accessing the point of operation, exposing employees to amputation hazards while operating the saw. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
