Maintenance personnel responsible for troubleshooting and/or performing electrical tests were exposed to electrical in that safe work practices were not employed to prevent electrical shock and/or other injuries.

29 CFR 1910.333(a)(1): Live parts to which an employee may be exposed were not deenergized before the employee worked on or was near to them: a. Pallet World, Inc. - Perrysburg, Ohio: On or about March 25, 2014, an employee was exposed to electrical hazards in that the main electrical power distribution panel was not deenergized and locked out before employees accessed the downstream bus bar to perform tasks such as, but not limited to, verifying absence of energy and replacing fuses. A maintenance worker was exposed to 480 volts while working near live parts at the bus bar to replace fuses located in the electrical panel for band saws.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed: a. Pallet World, Inc. - Perrysburg, Ohio: On or about March 24, 2014, the employer did not assure that employees who perform electrical tests to troubleshoot and verify absence of energy are provided with and use personal protective equipment to protect against electrical hazards. A maintenance employee, using a multi-meter to troubleshoot circuits at the main 480 volt panel, was exposed to electrical shock and arc flash hazards in that flame resistant clothing, insulated gloves with leather gloves, hard hat, and arc rated face shield were not worn during the troubleshooting activities.
