29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, pinch points, rotating parts, flying chips and sparks: (a) Facility, 2252 University Drive, Lemont Furnace, PA: On or about 01/14/14 the Amada SPH-60 press was not guarded to prevent employees from potential amputation hazards. An opening between the ram and die punch measured approximately 1/2" and was not guarded, creating a pinch point. Employees punch parts as small as 4" x 9" and are exposed to potential amputations from the unguarded opening.

29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: (a) Facility, Welding Area, 2252 University Drive, Lemont Furnace, PA: On or about 01/14/14 a junction box that supplied power to an outlet for the Miller Aerowave cc AC/DC Hybrid Arc Welding Source did not have a cover in place.
