The employer had an industrial fan that was used in the facility for cooling purposes in the summer months that had the ground prong removed. The fan was plugged in and unplugged by employees at the facility.  The fan was located at the work area of Ms. Linsey Simmons the day of the inspection.  The employer stated that the fans are moved around and there were several employees who could have plugged-in or unplugged the industrial fan.
