29 CFR 1910.95(c)(1): A continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (n) was not instituted when employee noise exposures equaled or exceeded an 8-hour time-weighted average (TWA) sound level of 85 dBA: An effective hearing conservation program which included noise monitoring, audiometric testing of employees and training of employees as detailed in the standard was not instituted for the following employees: a) Two employees working in the parts grinding area were exposed to a recorded personal noise dosimeter sound level of 92 decibels during the 420 minute sampling period on 1/30/14. b) An employee working in the molding area was exposed to a recorded personal noise dosimeter sound level of 90 decibels during the 420 minute sampling period on 1/30/14.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour: Employees were exposed to fire and explosion hazards in that the oxygen and acetylene cylinders were stored together in the maintenance shop.

29 CFR 1910.1000(a)(2): An employee was exposed to an airborne concentration of particulates not otherwise regulated (PNOR) total dust listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 15 milligrams per cubic meter. The following employee was exposed to inhalation hazards of PNOR total dust in that their personal exposure exceeded the OSHA Personal Exposure Limit (PEL): An employee engaged in furnace relining was exposed to PNOR total dust levels in excess of the OSHA PEL of 15 milligrams per cubic meter during the personal sampling performed on 2/08/14. The employee exposure was 24 milligrams per cubic meter, 1.5 times the PEL.
