29 CFR 1910.23(a)(8): Every floor hole into which persons could accidentally walk was not guarded by either a standard railing with standard toeboard on all exposed sides, or a floor hole cover of standard strength and construction.  While the cover was not in place, the floor hole was not constantly attended by someone or protected by a removable standard railing:  Melt deck / Pouring area - A floor hole was not guarded to protect employees from tripping hazards created by walking-by or working in front of the main control panel.  29 CFR 1910.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment would be isolated from the energy source and rendered inoperative:    Production area - Employees performing servicing and maintenance operations were exposed to hazards associated with the unexpected energizing or start-up of the equipment such as but not limited to the 1973 Match - Blomatic Molding Machine, the 1982 Match - Blomatic Molding Machine, the Mullor Sand Distribution System, and the Inductotherm electric furnace.  Employer did not develop written program for control of potentially hazardous energy on equipment.    29 CFR 1910.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):    Production area - Employees performing servicing and maintenance operations were exposed to hazards associated with the unexpected energizing or start-up of the equipment such as but not limited to the 1973 Match - Blomatic Molding Machine, the 1982 Match - Blomatic Molding Machine, the Mullor Sand Distribution System, and the Inductotherm electric furnace.  The written specific procedures for control of potentially hazardous energy on these equipment were inadequate in that they are just a list of energy sources and their locations, there are no step by step specific and clear instruction procedures.    29 CFR 1910.19(c)(1) requires certification that the abatement of the above violation is complete.

The two-hand tripping devices will still possibly actuate while the operator and/or the helper hands were inside the danger zone.  29 CFR 1910.19(c)(1) requires certification that the abatement of the above violation is complete.
