At the time of the inspection, the Toyota fork truck, m/n 7FDU30, S/N 61675, was being operated while in disrepair. a) The horn on the fork truck was in disrepair. b) The back up alarm on the fork truck did not work. c) There was no fire extinguisher in place on the fork truck in case of fire.

29 CFR 1910.178(l)(2)(ii): Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. 1. At least five (5) operators of the Toyota Fork Truck, m/n 7FDU30, S/N 61675, did not receive a combination of formal instruction, practical training and exercises, and evaluation. The (Paul's Recycling, Inc.) was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1910.178(l)(2)(ii), which was contained in OSHA inspection number 315205914, citation number 01, item number 003 (a) and was affirmed as a final order on (March 2, 2011), with respect to a workplace located at 2911 Garwood Street in Sikeston, Missouri 63801.

29 CFR 1910.22(d)(1): In every building or other structure, or part thereof, used for mercantile, business, industrial, or storage purposes, the loads approved by the building official shall be marked on plates of approved design which shall be supplied and securely affixed by the owner of the building, or his duly authorized agent, in a conspicuous place in each space to which they relate. Such plates shall not be removed or defaced but, if lost, removed, or defaced, shall be replaced by the owner or his agent. 1. At the time of inspection, inside of the production area, there was a platform above the tool storage room with no load capacity (weight limit) sign in place. There were several boxes of business records stored on this platform. The employer accesses this platform once a year to either store or retrieve business documents.
