29 CFR 1910.106(b)(6): In locations where flammable vapors may be present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition. Sources of ignition may include open flames, lightning, smoking, hot surfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat: a) Reynolds Gas Well Pad, Jessup Twp, PA: The employer did not eliminate or control sources of ignition such as static sparks by bonding the filling system to the conductive parts of a 510 barrel frack water tank, exposing employees to serious injury from a fire and explosion hazard, on or about January 11, 2014. In the alternative: Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fire and explosion hazard caused by the presence of flammable liquids and gases and potential sources of ignition: a) Reynolds Gas Well Pad, Jessup Twp, PA: The employer did not eliminate or control sources of ignition such as static sparks by bonding the filling system to the conductive parts of a 510 barrel frack water tank, exposing employees to serious injury from a fire and explosion hazard, on or about January 11, 2014. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED
