29 CFR 1910.212(a)(1):   One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a)   Northwest corner of the fabrication shop.  Burr King vertical belt grinder, Model 760, Serial No. 34500, 1 1/2-inch belt.   On or about January 16, 2014, and at times prior thereto, employees were exposed to a caught-in injury from the lack of a guard near the point of operation while deburring metal parts.   b)  Machine shop.  Burr King vertical belt grinder, Model 760, Serial No. 26553, 2-inch belt.  On or about January 16, 2014, and at times prior thereto, employees were exposed to a caught-in injury from the lack of a guard near the point of operation while deburring metal parts.   c)  Machine Shop.  Powermatic drill press, Model 1200, Serial No. 9720V195.  On or about January 16, 2014, and at times prior thereto, employees were exposed to an amputation or injury from the lack of a guard near the point of operation while drilling 1/2-inch holes in sheets of metal.                                               d)  Lock Shop.  Numberall Stamp machine, Model 131A, Serial No. P375698.  On or about January 16, 2014, and at times prior thereto, employees were exposed to an amputation or injury from the lack of a guard near the point of operation while stamping numbers on metal locks and keys.

29 CFR 1910.179(j)(2)(iv):  Visual inspection daily, and monthly inspection of hoist chains with a certification record was not accomplished which includes the date of inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected:  Various locations throughout the Fabrication Shop, 29 gantry bridge cranes with hoist chains and running ropes, such as but not limited to, 300-lb. CM Brand, 1/2-ton Dayton, 1-ton Jet, and 5-ton Kone.   On or about January 16, 2014, and at times prior thereto, employees were exposed to the hazards of malfunctioning overhead crane hoist chains, while transporting sheet metal in the Fabrication Shop.

29 CFR 1910.179(m)(1):   A thorough inspection of all ropes was not made at least once a month and a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier for the ropes which were inspected was not kept on file where readily available to appointed personnel:   Various locations throughout the Fabrication Shop, 29 gantry bridge cranes with hoist chains, such as but not limited to, 300-lb. CM Brand, 1/2-ton Dayton, 1-ton Jet, and 5-ton Kone.  On or about January 16, 2014, and at times prior thereto, employees were exposed to struck by hazards from malfunctioning overhead cranes, while transporting sheet metal in the Fabrication Shop.
