29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: Squad 1, receiving area, 110 Volt/ 20 Amp 2 inch by 4 inch receptacle: On or about February 11, 2016, and at times prior thereto, employees were exposed to electrical shock hazards while stocking 1/8 inch in diameter by 12 to 20 feet in length metal round bar over a receptacle that had a 1 inch in diameter knockout missing.
